Birth Registration of any child is a basic identification document and right of any child in Pakistan. Birth Certificate and B-Form are two different and separate. Birth Registration of any child is must for B-Form Registration in Pakistan as per recent laws. CNIC will be issued from NADRA Pakistan at age of 18 years as per B-Form. There are multi uses and purposes of Birth Certificate i.e. Birth Registration is must for issuance of B-Form, Passport, Under 18 CNIC, CNIC (in age of 18 years), school, college, university admission, issuance of visas., embassy purposes, legal heirship, citizenship.

REQUIRED DOCUMENTS FOR CHILD BIRTH REGISTRATION:-

1. Original Birth Certificate of Hospital / Clinic / Nursing Home / Doctor (with by name stamp of Doctor).
2. Attested photocopies of Parents CNICs (marital status should be update from NADRA Pakistan)
3. Affidavit of Birth from mother or father. (specific format)
4. Affidavit from LHW/LHV/Midwife in case of Birth at Home along with Vaccination Certificate or Education Document (having date of birth)
5. N.O.C from from the Office of Deputy Commissioner in case of late birth registration (for more than 5 years)
6. Attested copies of Passport, Education document, B-form in case of late birth registration (more than 5 years)
7. Fill Birth Registration Application Form
8. Deposit of schedule Fee (normal or urgent)
9. Any document (if require)

IMPORTANT NOTE:-

• Birth Registration will be made in CDA/DMA/MCI /Municipal Authority or concerned Union Council’s jurisdiction area of hospital / clinic where child was born or jurisdiction of address mentioned at parents CNICs.
• Birth Registration in Islamabad can be made in CDA/DMA/MCI or concerned union council.
• CDA / MCI / DMA office is opened from 8:30 am to 1 p.m (from Monday to Thursday and 8:30 am to 12 pm (Friday).
• Launching of Mobile Apps will be expected for Birth Registration in future.
• Documents file will be prepared for this purpose.
• B-Form Registration will be applied after issuance of Computerized Birth Certificate.
• Passport can be obtained from Passport office after issuance of B-Form from NADRA Pakistan.
